Credit information request

Upon approval of this credit application, the customer’s account will transition from "Prepaid/Credit Card" to "Open Terms." Please be advised of the following:

• Primary Payment Methods: All future invoices under these terms are to be settled via Check, ACH, or Wire Transfer.
• Credit Card Surcharge: If a credit card is used to pay an open invoice balance, a 3% processing fee will be applied to the total amount.
• Remittance: Payment should be mailed to our Los Angeles office.
